1. Obtain the prescribed application form from the District Project office, KGBV on all working days or download it from www.boudh.odisha.gov.in or www.opepa.nic.in.
2. Fill the application in the prescribed format.
3. Attach all required documents and testimonials (attested xerox copies only).
4. Include two self-addressed envelopes of 23cm x 10cm with postage stamps worth ₹25/- each.
5. Attach three original color passport photos with self-signature on the back side.
6. Submit the completed application form to the "Office of the District Project Coordinator, KGBV" via registered post by 5 P.M. on December 30, 2025.
